Claim Form Part A is to be filled by the policyholder. It includes personal details, policy number, hospital information, and a declaration regarding the claim.
Claim Form Part B is to be completed by the treating hospital. It provides medical details such as diagnosis, treatment, admission and discharge dates, and doctor’s certification.
